    I am personally satisfied as to my youth's fitness to participate in this activity. HYAA strongly recommends that you seek professional medical advice if there is any doubt about your youth's fitness to participate in this activity.

  2. Volunteer Bond
    Effective this season HYAA requires the payment of a $100 work bond, at time of registration, for all families participating in this HYAA program. Each family must work a total of four (4) hours during the season in order to have the $100 refunded. Failure to work the mandated 4 hours will result in bond forfeiture. In the event of forffeiture, HYAA will use the proceeds of the bond to hire the personnel needed to provide your children with a fun and safe learning experience. Prior to and during the season you will be trained in the use of the game clock and keeping the score book if given that assignment

Relief Foundation		 HYAA will be donating a portion of each registration fee to The Holmdel Victims Relief Education Foundation. If you would like to make an additional donation, please specify the amount in the box to the right and include the amount with your payment. Thank you.

Submitting this registration form will reserve your child's spot for this season. However, the registration will not be processed until payment is received. If paying by check, please make it payable to HYAA and mail it to: HYAA, P.O.B. 49, Holmdel, NJ 07733. Please indicate your child's name on your check. If paying by credit card, you will be prompted for your credit card information via our secure payment form.
